I know he isn't technically a boss but the hardest enemy for me was the optional hunter with a Chikage in the Cathedral at the end of the crow lady's story. If he hits you with a bullet while lunging at him. Visceral Game Over.
Coming from playing Bloodborne just before TW3 I was extremely annoyied at first.
